Home Study Documentation

Before the first home study visit from your social worker you will be required to gather various documents to submit to the agency. These documents will take time to receive so you should give yourselves ample time to retrieve them before the first visit.

Required documents for a Texas adoption home study include:

  • Child abuse clearances for all adults in household
  • Criminal check clearances for household members 14 years of age or older
  • Physician Reports for all household members regardless of age (must be current within one year)
  • Letter from Medical Specialist (if applicable)
  • Therapy Letter (if applicable)
  • One photo of front and back of house
  • Copy of floor plans for your home including dimensions and purpose for each room (can be hand drawn)
  • Most Recent Income Tax Return (copy of first page of the 1040)
  • Financial Statement (included in application)
  • Insurance Contact Form and copy of the front and back of your health insurance card (included in application)
  • Birth Certificates for all individuals living in the home (i.e. children, grandparents, caretakers)
  • Marriage Certificate
  • Adoption Decree (if applicable)
  • Divorce Decree(s) (if applicable)
  • Employment Verification (letter form employer, W-2, paystub, etc.)
  • Copies of Driver’s Licenses (or state issued ID)
  • Copies of Social Security Cards
  • Discipline and Safety Policy Form
  • Pet Vaccinations (if applicable)
  • 5 Reference Letters (only one family reference can be used) [link to reference letter article]
