Home Study Documentation
Throughout the home study process, you will be required to gather various documents and upload them into your online account.
Required documents for a Texas adoption home study include:
- Child abuse clearances for all members of the household 14 years of age or older (we will obtain these)
- Criminal check clearances for household members 14 years of age or older (we will obtain these)
- Letter from Medical Specialist (if applicable)
- Physical Examination Form for all household members
- Therapy Letter (if applicable)
- One photo of front and back of house including the yard area
- Copy of floor plans for your home including dimensions and purpose for each room (can be hand drawn)
- Most Recent Income Tax Return (copy of first page of the 1040)
- Financial Statement
- Insurance Contact Form and copy of the front and back of your health insurance card
- Birth Certificates for all individuals living in the home (i.e. children, grandparents, caretakers)
- Marriage Certificate
- Adoption Decree (if applicable)
- Divorce Decree(s) (if applicable)
- Employment Verification (letter from employer, W-2, paystub, etc.)
- Copies of Driver’s Licenses (or state-issued ID)
- Copies of Social Security Cards
- Discipline and Safety Policy Form
- Pet Vaccinations (if applicable)
- 5 Reference Letters (only one family reference can be used)
- Autobiographies
